Please use this template to submit your Review Report for Georgia State University.  Typically, the report is submitted by the chair of the review committee.  Please provide a response in each section, although you are welcome to simply enter “Not applicable” where appropriate.  Please submit the completed form to Dr. John Duffield, Director of Assessment and Review, at duffield@gsu.edu
